Disability Self-Identification Approved for Three More Years

On January 31, 2017, the Office of Management and Budget (OMB) approved the Voluntary Self-Identification of Disability Form for an additional three years, making the new expiration date January 31, 2020. An updated version of the form can be found here: https://www.dol.gov/ofccp/regs/compliance/sec503/Self_ID_Forms/SelfIDForms.htm.

Contractors should replace hard copies of the form with the new version immediately. According to the Office of Contract Compliance Programs (OFCCP) website, electronic copies of the form may either be replaced in their entirety with the new version or simply be updated to reflect the new expiration date of 01/31/2020. There have been no changes to form other than the expiration date.
